Fees

The cost of medical care is alarming. We sincerely desire to keep our fees from rising. We are members of the Lakeland Care PHO and participate with their full panel of insurance carriers. Additionally, we participate with Blue Cross/Blue Shield and the Cofinity Network. These companies we bill directly but you must pay your co-pay at the time of your visit.

If we are not participating physicians with your insurance carrier, all office visits must be paid for at the time services are rendered. Medical insurance consists of an agreement between you and your insurance provider. At each visit, we will provide you with an itemized copy of the services your child has received which you may then forward to your insurance company for reimbursement. Please remember, your bill is your responsibility. It is up to you to submit the appropriate forms to your insurance company for reimbursement.

Please feel free to speak to us regarding any questions you have about our fees or any financial problems you may have. We do not wish to have any child deprived of our care due to unavoidable financial issues or temporary financial difficulties.
